A staff review of the above mentioned project was held on this date in order to
give tentative approval of plans to allow applicant, to begin earthwork, pier and
foundation work on this project.. These plans are teeing forwarded to the Planning
and Zoning Commission for consideration of a Conditional Use Permit /Site Plan
approval. so that the applicant can begin preliminary cons truct.ionjsite work. A
conditional use permit was granted by the Planning and Zoning Commission for the
1,:)cation of a hospital on this site in eariv 1984.
The following conditons must be met before site plan approval. can be granted:
(l; The Electric Department is tentatively planning to supply this project with a
dual feed, manual switchover service. There is an existing overhead line along
Hock Prairie Road, and a proposal is submitted for an overhead extension along the
westerly property line to a point where the service will go underground. Details
available from the Electric Department.
i3; Furnish Electric load data information as soon as possible so transformer can
bey c >rdered. It is possible thaf delivery time can be up to 6 months.

(4) Location of possible utility lines should be studied regarding impact on
location of heliopad.
'5) There: must be a fire hydrant located within 300 feat of all parts of all
buildings.
KW The entire building must be equipped with sprinkler systems.
(7) Furnish driveway dimensions.
(8) Ail canopy areas must, maintain a minimum of 14 feet in height.
4 Fire lanes will be designated by Fire Marshal when more complete Site plans
area received. Fire Department would prefer 360 degree access to the building.
(10) Coordinate water lines with the Superintendent of Water & Sewer.
`;11) The 6" water line which is proposed will not be sufficient.
12) All waterlines must be in easements.
11) An all weather surface is required to be located within 150 feet of the
building prior to bringing in any combustible materials on the site. A mi_nimunl
base of 6" crushed limestone will be acceptable as an all weather :surface for this
drive, as it is meant to accommodate emergency vehicles. Maintain a 20 foot
minimum width, with 20 toot clear turning radius on this drive.
(14) P.R.C. Drainage Statement provided; compliance. required.
15) It has been determined that minimum dumpster requirements will be 2 5-yard
dumpsters to be located on a pad at least 12 ft. x 20 ft., with a clear 50 foot
radius in front to accommodate service vehicles. Should compactors be used, a 12
foot minimum width area with the same clear 50 foot turning radius must be
provided.
(16) Revisions to plans must be submitted on 2036 inch sheets.
(17) On revised site plan, include the following information:
;a) Row -by -row parking space count; total shown on legend.
(b) Show island dimensions (9'x 20' minimum size).
0 Show dimensions of all drives.
A ' ) Furnish the approximate total employee count per shift.
(e) Mark handicap parking spaces, using State requirements to determine
required number.
(0 Include all signage information.
(g) Typical parking row - drive- parking row dimensions are 20- 23 -20, with 18
foot: spaces allowed on perimeter rows with 2 foot overhang.
x:18) Landscaping plans, which can be on a separate 2406 inch sheet can be phased
if completion of entire project is not planned at one time. Mark phases lines and
calculate required landscaping accordingly. Include all information required by
ordinance. Show location and type of irrigation system planned.
SUBMIT 10 COPIES OF REVISED SITE PLAN TO PLANNING DEPARTMENT PRIOR TO NOON ON
