hereby, on oath, state the followg: I have a substantial interest in a business entity or in real property that may
receive a special economic effect that is distinguishable from the effect on the public as stated in Texas Local
Government Code Chapter 171.
The business entity r real p4roi4v I have a substantial interest in is:

and the reason I or a person that is related to me within the first degree of consanguinity (blood) or affinity
(marriage) have a substantial interest in the above is for the following reasons: (Check all that apply.)
I own 10% or more of the voting stock or shares of the business entity;
I own either 10% or more or $15,000 or more of the fair market value of the business entity;
Funds received by me from the business entity exceed 10% of my gross income for the previous year; Real
property is involved and I have an equitable or legal ownership with a fair market value of $2,500 or more;
A relative of mine has a substantial interest in the business entity or real property that would be affected by
a decision of the public body of which I am a member.
Alternatively, even if I do not have a "substantial interest" as defined by Chapter 171 of the Texas Local
Government Code, I am filing this affidavit so to avoid the appearance of impropriety. My interest may be
