the possibility of soil and /or groundwater contamination on -site is pertinent to these
assessments. The environmental assessment is being performed on the following

As part of this assessment, I am requesting information concerning this property
from the following City departments concerning records or knowledge of the
following:
From the Health or Environmental Department: Any environmental code violations
or concerns associated with the facility such as accidental discharges, spills,

From the Building /Planning Department: Building permits (including the original, if
available), and certificates of occupancy
