The Permittee is responsible for all associated with the construction of this access driveway. 2. Design of facilities shall be as follows and/or as shown on sketch and is subject to conditions stated' below; Provide a 33' wide driveway in ditch section of Hwy 40 with 67' ingress and egress radius. t - -- All construction and materials shall be subject to inspection and approved by the State. 3. Maintenance of facilities constructed hereunder shall be the responsibility of the Permittee, and the State reserves the right to require any changes, maintenance or repairs as may be necessary, to provide protection of life or property on or adjacent to the highway. Changes in design will be made :only with approval of the State, 4. The Permittee shall hold hatmless':the State and Rs duly appointed agents and employees against any action for personal injury or property damage sustained by reason of the exercise of this permit. 5. Except for regulatory and guide signs at county roads and city streets, the Permittee shall' not erect any sign on or extending over any portion.of`the highway right of way, and vehicle service fixtures such as fuel pumps, vendor stands, or tanks shall be located at least 12 feet from the right of way line to ensure that any vehicle services from these fixtures will be off the highway right of way. 6. The State reserves the right to require a new access driveway permit in the event of a land use change or change in driveway traffic volume or vehicle types. 7. This permit will become null and void if the above - referenced driveway facilities are not constructed within six (6) months from the issuance date of this permit. 8. The Permittee will contact the State's representative �'T ay �STV telephone, (9`7[ )11&- ., at least twenty - four (24) hours prior to beginning the work authorized by this permit. For this purpose, the booklet entitled "Regulations for Access Driveways to State Highways ", was published and adopted, setting out departmental policies to regulate construction and maintenance of access driveway facilities. Sketch of Installation SH 40 TOWER POINT DEVELOPMENT Access Driveway (# 4) + Decel Lane - See Attached Provisions (Important) - September 10, 2010 RE: SH 40 (North ROW) East (800') of Arrington Road Tower Point Development College Station, Texas Access Driveway (1) Permit - W # 4 (H) = 34' B -B w/ Decel Lane + 18" x 164' RCP w / SET's GENERAL SPECIAL PROVISIONS (Access Driveway Permit) : 1) Traffic Control plan is required per TMUTCD prior to start of construction work in the SH 6 & SH 40 right -of -ways. 2) Areas within ROW disturbed by construction shall be restored to equal or better conditions than existing prior to construction. 3) Contractor shall be responsible for locating all utilities within the right -of -way construction site. Contacts with local utility companies and utility locators are required. 4) Any lane closure will be allowed between 8:30 AM and 4:30 PM only. 5) Construction for connecting concrete driveway shall be as indicated for City of College Station Commercial Driveways & Plan Details. 6) Decel lane is required A the SH 40 Access Driveway locations.(see attached) 7) Pavement section for decel lanes shall be as shown (see attached) Sheet C5.2). 8) Curb & Gutter and RFC apron construction shall be in accordance with City of College Station Standard Details & Specifications & Plan Details . (See Attached) (R = 67') 9) Raised curb (6 ") shall not be constructed within 30' of existing travel lane white stripe (SH 40). 10) No undercutting of existing asphalt pavement will be allowed. Existing pavement on SH 40 the entrance site shall be removed by neat cutting as indicated. Pavement tie -ins shall be as per plan details. 11) Ditches (upstream & downstream) a roadway access drives, etc. shall be graded to drain as indicated. 12) SET's (6:1) on pipe ends shall be required as indicated. (see attached). Rock - rubble riprap (grouted) shall be placed as indicated. 13) Contractor shall notify Norman Maurer Brazos Maintenance (778 -8054) for handling of any roadway signs in conflict with access driveways & decel lanes construction. 14) All signs installed by the Contractor in the SH 6 ROW shall have breakaway bases as per TxDOT Standard Details. 15) Decel lane pavement markings, etc. shall be as per plan details or as per TMUTCD; widths of existing roadway travel lanes shall remain unchanged. Decel lane edge line = 8" wide along SH 40. 16) Pavement surfaces shall be kept clear of mud, debris, etc. 17) Contractor shall contact Mr. Walt Norwood @ 778 -8054, TxDOT, or WNorwood @ Dot.State.Tx.US , 48 hours prior to starting work. NOTES: Pavement painted raised (4") triangular divider is /and shall be placed Access DW #4 (H). Face of island shall be a minimum of 24" north of decel lane edge fine.
