This letter and enclosed information is for your records concerning the soil

We applied one(1) gallon of chemical to every ten(10) square feet of slab area.
We applied four(4) gallons of chemical per ten(10) linear feet of the perimeter.

Please see attatched label and consumer information sheet for additional
information.
Our work is guaranteed for a period of five(S) years frome the date of initial
treatment, with inspections made annually at a cost of $ plus tax
per year. Should you decide not to carry the contract, the initial treatment is
guaranteed only for one (1) year from date of treatment. Should any
reinfestation occur during the term of the contract, we will retreat at no
additional charge, providing that the contract is kept current. The contract is
transferable to new owners if the property is sold during the life of the contract
and all annual inspections are current. Exclusion: Accurate Termite & Pest
Control will not be held responsible for any "previous" or "future" structural
damage. If you wish to carry the contract, please sign the copy of the attatched
contract and return it to my office.
The guarantee will be void if the soil is disturbed after the chemical has been
applied. For best results in preventing reinfestation, do not stack wood against
the foundation.
The cost of treatment wasS per square foot. Total cost was

For all treatments there will be a diagram showing exactly what will be treated. Treatment specifications and warranties for those treatments may vary widely. Review the pesticide
label provided to you for minimum treatment specification. Remember that your architects or design engineer's specifications must also be followed, unless they are in violation of the law.

Before conducting a termite pre -construction treatment, the company will present a complete diagram of the structure including construction details. When construction prevents
performance of a full treatment, in accordance with a bid for full treatment, any change to a partial treatment by the company providing treatment will be permitted if the owner of the
structure or the person in charge of the construction and the certified applicator for the pest control company sign a statement attesting to the construction conditions. The
agreement must be attached to the contract with an amended diagram showing the exact areas to be treated. Copies must be sent to the owner of the property and to the Structural
Pest Control Board within seven days of the application.
