5 Year Subterranean Term gRP Lee Termitc and Pwq Control will retreat the structure for Subterranean
Termites at no cost to the customer, if an infestation of Subterranean Termites is found, as Iona as the original trzatrnent is paid in full
and the customer is current on their yearly renewals. This Agreement will expire in one (1) year unless the customer elects to pay
the yearly warranty fee. The guarantee can be renowcd for up to five (5) years by paying the yearly warranty fee. RP Lee Termite
and Pest Control may approve additional warranty years after the five (5) years with an additional increase in the yearly warranty fee.
The guarantee does not cover any damages to the structure or contents of the customer. The customer agrees to control all conducivc
conditions that may cause rcinfestation. If the structure is modified, added on to, altered, or if the soil treatment around the structure
is removed the customer must contact RP Lee Termite and Pest Control and pay for additional treatment in order to keep the guarantee
in effect .If the customer does not pay for additional treatment the retreatment guarantee can be cancelled °°
ReltewaLFea: Customer may elect to renew the rctrcatment guarantee for addition years. The renewal fee is S �0, plus tax
The renewal fee is due on or before the annual anniversary date or paid $� phrs tax every third (3) month with inspection.
The customer agrees to correct all conducive conditions, at owner's cxpame, as they exist or as they
occur in the future in order for the guarantee not to be cancelled. Customer will make the structure available for inspections and
treatments as requested by RP Lcc Termite and Pest Control.

and Pest Control deems necessary. RP Lee Termitc and Post Control recommends that the customer call and sct up an aanaal inspec-
tion at the time of customers yearly renewal.

binding, non -appealable and may be entered and enforced in any court having jurisdiction in accordance with the Federal Arbitration
Act
Umltstion of LIabllity: Landscaping and grass next to the slab may be damaged during treatment, carpet may creed to be pulled
up in certain areas around the home, drill holes may need to be trade in floor coverings such as the or linoleum or in cabinets next
to pipe areas, and holes may need to be cut in walls. Customer agrees not to hold RP Lee Termite and Pest Control liable .for these
damages, or treatment techniques, or other damages that may occur during treatment.
AUUMML This Agreement shall be the entire agreement between customer and RP LeeTermitc and Pest Control. If any part of
this agreement is found to be invalid it shall not affect the validity of any other part of the Agreement.

diagram showing the exact areas to be treated. Copies must be sent to the owner of the property within seven days of the application.
