5 Year Subterranean Tertatte Retr®atment I1 neAlltePt RP Lee Termite and Pest Control will retreat the strt cb= for Subterranean
Termites at no cost to the customer, if an infestation of Subterranean Termites is found, as long as the original treatment is paid in full
and the customer is ct> cnt on their yearly renewals. This Agreement will expiM iu one (1) year unless the customer elects to pay
the yearly warranty fee. The guarantee can be renewed for up to .five (5) years by paying the yearly warranty fee. RP Lee Termite
and Pcst Control may approve additional warranty years after the five (5) years with an additional increase in the yearly warranty fee.
The guarantee does not cover any damages to the structure or contents of the customer. The customer agrees to control all cortdueive
conditions that may cause reinfestation. If the structure is modified, added on to, altered, or if the soil treatment around the stateturo
is removed the customer must contact RP Lee Termite and Pest Control and pay for additional treatment in order to keep the guarantee
in effect If the eustozrtct dots not pay for additional treatment the retreatment guarantee can be cancelled. Vu
Renewal Fee: Customer may elect to renew the retreatment guarantee.£or additional years. The renewal fee is S .I �� v10 ta.
The renewal fee is due on or before the annual anniversary datc or paid S / plus tax every third (3) month with inspection.
The customer agrees to correct all condusivc conditions, at owner's expense, as they exist or as they
occur in the future in order for the guaranteo not to be cancelled. Customer will make the structure available for iztsprctions and
treatments as requested by RP.Lec Termite and Pest Control.
$gjnspection: RP Lee Termite and Pest Control will reinspect the structure at customers roqucst or any other times RP Lee Termite
and Pest Control deems ncccssary. RP Lee Termite and Pest Control recommends that the customer call and set up an annual inspec-
tion at the time of customers yearly renewal
This agreement may be transferred to anew owner with the payment of a transfer fee of S , copy of the Wood

above agree to participate in mediation through and in accordance with the commercial mediation rules of the American Arbitration
Association. The Arbitrator shall consider the legal defenses raised in the arbitration, and the decision of the arbitrator shall be final,
binding, non -appealable and may be entered and eaforccd in any court having jurisdiction in accordance with the Federel.A.rbitration
Act.
Limitation of L.iahLlhy Landscaping and grass next to the slab may be damaged dozing treatment, carpet may need to be pulled
up in certain areas around the home, drill holes may need to be made iu floor coverings such as tilt or linoleum or in cabinets next
to pipe areas, and holes may need to be cut in walls. Customer agrees not to hold RP Lee Termite and Pest Control liable for these
damages, or treatment techniques, or other damages that may occur during treatment.

the construction and the certified applicator for the pest control company sign a statement attesting to the construction conditions. The agreement must be attached to the contract with an amended
diagram showing the exact areas to be treated. Copies must be sent to the owner of the property within seven days of the application,
