,5 Xg$r Subtutamix Termite Retreatmggt< guarat RP Lee Termite and Pest Control will retreat the structure for Subterranean
Termites at no cost to the customer, if an infestation of Subterranean Termites is found, as long as the original treatment is paid in full
and the customer is current on their yearly renewals. This Agreement will expire in one (1) year unless the customer elects to pay
the yearly warranty fee. The guarantee can be renewed for up to five (5) years by paying the yearly warranty fee. RP Lee Termite
and Pest Control may approve additional warranty years after the five (5) years with an additional increase in the yearly warranty fee.
The guarantee does not cover any damages to the structure or contents of the customer. The customer agrees to control all conclusive
conditions that may cause reinfestation. If the structure is modified, added on to, altered, or if the soil treatment around the structure
is removed the customer must contact RP Lee Termite and Pest Control and pay far additional treatment in order to keep the guarantee
in effect. If the customer does not pay for additional treatment the retreatment guarantee can be cancelled
Renewal Fee: Customer may elect to renew the retreatmcnt guarantee .for additional years. The renewal fee is S .10, v�,
Dlus tax.
The renewal fee is due on or before the annual anniversary date or paid S /' plus tax every third (3) month with inspection.
Maintalninn the Guarantee: The customer agrees to correct all conclusive conditions, at owner's expense, as they exist or as they
occur in the future in order for the guarantee not to be cancelled. Customer will snake the structure available for inspections and
treatments as requested by RP Lee Termite and Pest Control.
ReinsgSggn, RP Lee Termite and Pest Control will reinspect the structure at customers request or any other times RP Lee Termite
and Pest Control deems necessary. RP Lee Termite and Pest Control recommends that the customer call and set up an annual inspec-
tion at the time of customers yearly renewal.
@$ga; This agreement may be trsosferred to anew owner witb the payment of a transfer fee of $ copy of the Wood
Destroying Insect Report from the sell, and approval of RP Lee Termite and Pest Control.
This Subterranean Termite Retreatmenj Guarantee dom not prevent and is not iaten ed to control other form of ermites
(Drvwoad Formouan tc.l. Wood Boring Beetles CaMenter-AWL Carpenter Bees. Woo&Rat. Fungus. Decay, or any other
wood destrovipg insects or oMMIM
Arbitration; In the event of soy dispute arising out of this agreement or any other services perforated or not performed the parties
above agree to participate in mediation through and in accordance with the commercial mediation rules of the American Arbitration
Association. TheArbitrator shall consider the legal defenses raised in the arbitration, and the decision of the arbitrator shall be final,
binding, oon-appealable and may be entered and enforced in any court having jurisdiction in accordance with the Federal Arbitration
Act.

up in certain areas around the home, drill holes may need to be made it floor coverings such as We or linoleum or in cabinets next
to pipe areas, and holes may need to be cut in walls. Customer agrees not to hold RP Lee Termite and.Pest Control liable for these
damages, or treatment techniques, or other damages that may occur during treatment.
I: This Agreement shall be the entire agreement between customer and RP LeeTermite and Pest Control. If any part of
this agreement is found to he invalid it shall not affect the validity of any other part of the Agreement.
