All samples were analyzed by Polarized Light Microscopy (PLM) in our TDSHS licensed laboratory (TDSHS License No. 30-0007) and found to be non asbestos -containing. ERI Consulting, Inc. is a TDSHS licensed asbestos consulting agency (TDSHS License No. 10-0020). Current EPA and Texas Department of State Health Services regulations require that an asbestos survey be performed prior to disturbing building materials (demolition or renovation) in a public building. TDSHS regulations require that a notification be filed only if structural members are removed, or asbestos -containing materials are disturbed. TDSHS does not require nor desire copies of the survey report to be filed with them. TDSHS regulations require that a copy of the asbestos survey be kept on the job site at all times during renovation activities. Senate Bill 509 requires facility owners to provide proof of asbestos survey to city building officials prior to building permits being issued. The attached report should assist you in complying with both of these requirements. In summary, no asbestos -containing materials were found in the Wal-Mart Store #1150,1815 Brothers Blvd., College Station, Texas. PROJECT OVERVIEW ERI Consulting, Inc. of Tyler, Texas, was commissioned by Ms. Teri Hite, of Wal-Mart Stores, Inc. #8085 during November 2004 to conduct an asbestos inspection of the Wal-Mart Store #1150, 1815 Brothers Blvd., College Station, Texas. The inspection was performed in accordance with applicable TDSHS, OSHA, and EPA standards. The purpose of the inspection was to determine the location of the asbestos -containing materials used in the construction of the facility. The inspection was performed by Mr. Christopher L. Power and Mr. Todd W. Davis of ERI Consulting, Inc., licensed through the Texas Department of State Health Services for inspections, TDSHS License No. 10-5566 and 60-0872. The field inspection was performed on November 29, 2004. During the inspection the facility was occupied. Access to all areas of the building was provided by the Store Manager. The inspection involved the collection of 54 bulk samples, representing 18 homogeneous materials. The initial phase of the inspection involved the identification of the homogeneous areas suspected of containing asbestos fiber. Each homogeneous area was quantified to determine the approximate amount of suspect materials present. These areas were then divided into sectors and samples collected in a statistically random manner. A sufficient number of samples were collected in each homogeneous area based on the amount of suspect material present. iAll samples collected during the , inspection were delivered to the laboratory of ERI Analytical immediately following the field inspection. Suspect asbestos -containing samples were analyzed by Polarized Light Microscopy (PLM), in accordance with 40 CFR 763 Subpart F Appendix A. This report contains information documenting the field inspection and laboratory findings. Such materials, if present, would be located in enclosed or concealed pipe chases, behind walls, or other inaccessible areas.
