5 Year Subterranean Termite Rerreatment Guarantee: RP Lee Termitc and Pest Control will retreat the structure for Subterranean
Termites at no cost to the customer, if an inficstation of Subtctnmean Termites is found, as long as the original trcatmeut is paid in full
and the customer is cmneut on their yearly renewals, This Agreement will expire in one (1) year unless the customer elects to pay
the yearly warranty fee. The guarantee can be renewed for up to five (5) years by paying the yearly warranty fcc, RP Lcc Tcrmitc
and Pcst Control may approve additional warranty years after the five (5) years with an additional increase -in the yearly warranty fee.—
Theguarantee does not cover --
any damages to the structure or contents of the customer. The customer agrees to control all conclusive
conditions that may cause reinfestation. If the structure is modified, added on to, altered, or if the soil treatment around the structure
is removed the customer mv_et contact RP Lcc Tcrmtitc and Pcst Control and pay for additional trcatmcnt in order to keep the guarantee
in effect If the customer dots not pay for additional treatment the rctrcatmcnt guarantee can be cancelled. 04
Renewal Fee: Customer may elect to renew the retreatment guarantee for additional years. The renewal fcc is $ J'O, vlus tax.
The renewal fcc is duc on or before the annual anniversary date or paid S / plus tax every third (3) month with inspection.
Mai„tsilliilg Shg rzMarantee: I The customer agrees to correct all conducive conditions, at owner's expense, as they exist or as they
occur in the future in order for the guarantee not to be cancelled. Customer will make the structure available for inspections and
treatments as requested by RP Lee Termite and Pest Control.
Reins cos tion: RP Lee Termite and Pcst Control will reinspect the structure at customers request or any other times RP Lee Termite
and Pest Control deems acccssary. RP Lcc Termite and Pest Control rccommends that the customer call and scl up an annual iubpm-
tion at the time of customcrs yearly renewal,

In the event of any dispute arising out of this agreement or any other services performed or not performed the parties
above agrco to participate in mediation through and in accordance with the commercial mediation rules of the American Arbitration
Association, The Arbitrator shall consider the legal defenses raised in the arbitration, and the decision of the arbitrator shall be final,
binding, non -appealable and may be entered and enforced in any court having j urisdiction in accordance with the Federal Arbitration
Act.
Limitation of Liability: Landscaping and grass next to the slab may be damaged during treatment, carpet may need to be pulled
up in certain areas around the home, drill holes may need to be made in floor coverings such as tilt or linoleum or in cabinets next
to pipe areas, and holes may uccd to be cut in walls. Customer agrees not to hold RP Lee Tern -Ate and Pest Control liable for these
damages, or trcatmcnt techniques, or other damages that may occur during treatment
AeMjMegB This Agreement shall be the entire agreement between customer and RP LeeTermite and Pest Control. If any part of
this agreement is found to be invalid it shall not affect the validity of any other part of the Agreement.

This drawing needs the Fully Sheathed note on it, the right side entry to garage either needs to be
modified to 27" min. OSB for regular framing or 18" min. APA Framing, and also Master/Owners Retreat
wall needs 27" min. OSB, it may have it but is too close to measure on the small scale 1/8" drawings.
Also due to the high glazing percentage the attic insulation needs to be R-38 not R-30. The spans of the
ceiling joists shown on the framing details exceed span in many instances.
Please correct these items and resubmit.
