To Whom it May Concern:
Please find attached the following information and construction drawings for the above
referenced project. These documents are submitted for your review and commenting.
2 - Grading & Erosion Control Plan;
1 - Development Permit Application (Boardwalk at Dominik, LLC):
• $600.00 Development/Infrastructure Inspection Fee;
• Proof of Signing Authority (Boardwalk at Dominik, LLC);
Below are the nine (9) bullet items, with the appropriate response, that should be addressed with
the issuance of a D.P. in a Special Flood Hazard Area (SFHA).
A comprehensive site plan will be required prior to issuance of a development permit to clear and/or fill on
this site - as per the Ch. 13 Flood Hazard Protection Ordinance, Section 4.E:
(4) The compatibility of the proposed use with existing and anticipated development; The redevelopment
of this lot will be to replace the existing duplex structure and driveway with new facilities;
(9) The relationship of the proposed use to the comprehensive site plan for that area. " No comprehensive
site plan for this area exist;
"Comprehensive "further underscores the requirement of the site plan to be complete meeting all requirements
of the UDO and those outlined on the application. Note that an individual single family lot may provide a lot
layout with building footprint, driveway, etc. to satisfy the site plan requirement in lieu of a complete building
permit application. The redevelopment of Lot 20 is residential in nature. Therefore a lot layout (building
footprint, driveway, etc...) will be submitted in lieu of the UDO comprehensive site plan.
Applicant must submit a Development Permit Application and fees. Special attention should be given to the
Engineer's Certifications associated with the floodplain and floodway. A Development Permit application
and fees is included with the submittal package for this project along with the appropriate Engineer's
certifications;
3. Applicant must submit an Engineer's or Surveyors' topographical map depicting: The applicable
information, listed below, is contained on the Grading & Erosion Control Plan.
• the current floodplain and if applicable, the floodway on the site
• limits of proposed fill area within the floodplain and if applicable, center line of the floodway

4. Applicant must submit a grading plan with pre-grading contours, post-grading contours, and depiction of
limits of proposed fill area. The applicable information, listed above, is contained on the Grading &
Erosion Control Plan.
5. Applicant must submit an Erosion Control Plan describing and depicting pre and post construction erosion
control measures protecting the adjacent properties, especially the adjacent floodplain and floodway. The
applicable information, listed above, is contained on the Grading & Erosion Control Plan.
6 Applicant must submit proposed minimum finished floor elevation based on the Base Flood Elevation (BFE)
from the current Flood Insurance Study (BFE depicted on the Flood Rate Insurance Map (FIRM) is not
acceptable). Minimum finished floor will be established in accordance with the standards set forth in the
Unified Storm Water Design Guidelines, Section IX, Appendix B, and Table B-2 Minimum Floor Elevations
along Selected Named Regulatory Watercourses. The minimum finish floor elevations (291.00'), as stated
on the Replat, is based upon the FEMA approved LOMR Case No. 08-06-1882P with an effective date of
December 12,2008;
7. Applicant must demonstrate that fill in the floodplain is "reasonably safe from flooding. " Adequate
compaction efforts are required to ensure fill does not erode or sink. FEMA Technical Bulletin (TB) 10-01
provides example guidance and an Engineer's Certification of Compaction of fill. Note that:
A. If applicant is planning to apply for a LOMR-F, an Engineer's Certification of Compaction of fill in
accordance with FEMA TB 10-01 will be required. Not applicable for this project;
B. For Building Permits with foundation beams that do not extend into the natural soil a minimum of 6
inches the requirement will include either Engineer's Certification of Compaction of fill to 95%
Standard Proctor (ASTM D 698) between + 2 of optimum moisture content, or an Engineered Slab
Plan. Foundation Plans & Details will illustrate either appropriate beam depths or compaction
fill efforts;
8. Applicant must submit FEMA Form 81-31 Elevation Certificate (only for buildings that encroach in to the
Special Flood Hazard Area) during the construction phase prior to pouring the minimum finished floor
(Building Under Construction). An Elevation Certificate, for both construction and final phases, will be
supplied for the Lot 20 redevelopment;
9. Applicant must submit FEMA Form 81-31 Elevation Certificate after completion of the building (Finished
Construction). The finished construction elevation certificate must be completed in accordance FEMA Form
81-31 Instructions and approved by the City Engineer before a Certificate of Occupancy will be issued. An
Elevation Certificate, for both construction and final phases, will be supplied for the Lot 20
redevelopment;
