Anchor Piers LLC, called Contractor, and MNZ K -1.)A'Y , Owner agree that the sum
of $ 000 , to be paid "as follows: 5o after piers are poured, balance on completion.
Contractor will furnish labor, equipment and materials to perform the following described work to the hereinafter described
building or structure located at: /60 Z 6>Cca2Se 3usrV DR. Story YL,4e Structure

1. The work shall consist of the installation of lep piers with nine inch diameter shafts drilled to 8" to 12' , water
table rocks and/or man placed objects permitting. The bottom of the shaft will have a bell footing of 20 inches in
diameter, unless rock is encountered.
2. Contractor assumes no responsibility for any under slab plumbing. It may currently be cracked, broken, deterio-
rated or may do so in the leveling process. Any repairs, if required are the sole responsibility of the owner.
3. Job site will be clean of all excess dirt and material.
4. Contractor will carry all required insurance.
5. When inside piers are called for, contractor will patch floors. Patches will be made with concrete to wooden floor
level. Carpet will NOT be stretched.
6. Where surfaces such as concrete floors or patios are broken in order to install piers, Contractor will patch such
area. The patches, however, will not match he color or texture of the original material. Special surface such as
Flagstone, Spanish Tile, Brick or other such special materials will be restored at a cost in addition to pier installa-
tion.
7. In foundation leveling, conditions might appear when work is begun that were not visible when estimate was
submitted, such as, insufficient reinforcing steel in concrete slab, and/or other original foundation structural
deficiency. In such cases the guarantee becomes invalid.
8. SPECIAL PROVISIONS: No Subcontractors.S
9. LIFETIME SERVICE AGREEMENT: If any raising is necessary due to settling during the first five years from date
of completion of the job, Contractor will re -raise the settled area without cost to the OWNER. For additional
years, servicing, if required, will be done at the rate of -SO '0C. per adjusted pier, labor and materials included,
plus or minus the U.S. Consumer Price Index each year. This service agreement applies only to the _ / G piers
installed by Contractor.
THIS AGREEMENT TRANSFERABLE TO NEW OWNERS. NOTIFICATION MUST BE TENDERED IN WRITING
WITHIN 30 DAYS OF OWNERSHIP.
It is understood agreed that in order to perform the above described work it is possible that sheetrock, wallpaper, or other
rigid material may crack. Therefore, the above figure does not include any redecorating, repairing, electrical work or the
replacement of any material not called for in this agreement. Contractor upon request from owner, will transplant plants
and shrubs obstructing operations, and replace same, survival of shrubs and plants cannot be guaranteed.
It is understood and agreed that Contractor will furnish all the labor, equipment and material and will perform all necessary
work in connection with this job, in a good workmanlike manner.
The Owner may order extra work to be done not contemplated by the contract. In which event, a separate Agreement for
such work shall be entered between the Owner and the Contractor. No oral representation made by anyone can change
or modify this Agreement. This Agreement in order to be binding by both parties, must be signed in the space provided
below, and the original copy returned to this office within -30 days from the date shown above.
