LABORATORY ANALYSIS METHOD:
Summary of polarizing light microscopy (PLM / stereomicroscopy bulk asbestos analysis)using the
methods described in 40CFR Part 763 Appendix E to Subpart E (Interim) and EPA /600/R-93/116
(Improved). All analysts have received the necessary in-house and extramural training McCrone
Research and/or University Degree in Geology, Biology, Environmental and Material Science) to
perform analysis of bulk samples for the presence or absence of asbestos. Greater than on percent are
re-examined by a second analyst for intralaboratory quality control. Greater than one percent are re-
examined by the same analyst for quality control. All analysts are required to participate in quality
control analysis rounds. Microscopic calibrations are performed on a daily, weekly and monthly basis.
Some samples (floor tiles, surfacing, etc.) may contain fibers too small to be detectable by PLM. All
asbestos qualification is traceable to NIST standards for regulated asbestos types. Analysts' calibrated
visual estimated percentages are susceptible to variance. All quantifications fall within a range of
acceptable percentages, depending on the actual concentration of asbestos:

Hi Tom,
I visited the proposed P.T. site this afternoon. There are a couple of things I wanted to pass on to you
before your meeting with John Clark.
1. There is an' existing water fountain on the wall in the proposed office area of the PT/LAT office (room #
112). If possible, we would like to`remove this, cap the water lines and have this as office wall space. We
included a bottle style water dispenser on our equipment list. We would like the requested water
cooler-dispenser/with cups to be located in the gym area for patient use when they are exercising. This is
more efficient placement and is more sanitary for patient use than the existing one.
2. We would like to femdve the dishwasher (kitchen area) and utilize that under counter space for an
undercounter style refrigerator. We would like to keep the remainder of the counter/cabinet space in the
kitchen area as is. We will utilize those cabinets for any additional storage for office supplies, etc if
needed. Cherie Sajewski is helping us with the refrigerator search. Staff is going to purchase a small
microwave, coffee pot, etc. We just need to insure that the electrical supply is adequate in the kitchen
regarding refrigerator, etc.
3. The,PTA Office #113 currently exists as a 10x12 office. I have spoken with Debbie, she agrees we
should leave' it as it currently exists. She does feel we need to either make the existing window one way
mirrored glass or eliminate the window completely for confidentiality purposes.
