SUBJECT: 909 SOUTHWEST PARKWAY E.
I reviewed the above-mentioned PRIVATE IMPROVEMENT IN ROW PERMIT (PIP)
application turned in for today's deadline and determined it to be incomplete. The
following is the preliminary list of items needed to complete the submittal so that staff
may conduct a thorough review. Please submit the following information by10 a.m. on
any Monday for review that week:
1" JT A signed and notarized indemnification form. Since the pMoperty owner is
an organization, the form needs to be accompanied by proof of authority
for that individual to sign the document on behalf of the organization.
Four (4) copies of the facility layout plan. While the existing and
proposed landscaping elements were shown, the following are also
needed on the plan:
• Location, identification, and dimensions of the rights-of-way and the
curbs.
• Location of all existing public utilities-dimensioned from the right-of-
way or back of curb and stating the materials and sizes.
The latest plat or site plan of the property may assist with this information.
Please be aware that if this application is not completed before October 4, 2010, it will
expire and a new application and fees will be necessary to continue the private
improvement in the public right-of-way approval process.
