Receipt for filing fee ($150.00 cr $75.00 for in-house child care - min. 7 children). to Sixteen (AIS) copies of site plan which includes requirements for site plan proposals as listed on attached sheet. This site plan will go to P.R.C. for review, after which thirteen (0) copies of revised site plan will be required. 10 3. Typed on a different sheet of paper (See the back page). 4. If sign is planned for project, site plan should include sign elevation and details. If sign details are not included on plans submitted with this application, any future sign may have to be considered as a separate conditional use permit and all of the above requirements must be repeated. By carrying this plan into effect, traffic impacts are expected to be minimal. 4. Protection of neighborhoods Adequate outdoor security lighting to provided, including outdoor building lighting and parking lot lighting. 3. (1) Facility to be used for church meeting location. (2) Currently there are approximately 160 attendees(125 adults & 35 children) on Sunday morning Service with approximately 50 vehicles. (3) Currently the church meets Sunday Morning(2 hours), Wednesday Night(2 hrs) and Friday Night(2 hours). (4) The church currently has one employee. CHURCHES Minimum Requirements: 1. This site plan will go to P.R.C. for review, after which thirteen (13) copies of revised site plan will be required. 3. Detailed explanation of proposed use including hours of operation, anticipated traffic, number of employees, number of students, children, church members, etc., as applicable. To be determined following the regulation of City. APPLICATION WILL NOT BE CONSIDERED COMPLETE WITHOUT THE FOLLOWING INFORMATION ADDRESSED: 1. Approximate the distance to the nearest night clubs, restaurants, and retail commercial establishments. There are no night clubs, restaurants, and retail commercial establishments in the immediate vicinity of this location. 2. Discuss the possible necessity for screening (fence or vegetation) between the subject site and adjoining residential properties. This property adjoins a City Park(Sandstone) on the East, a Holy Cross _Lutheran Church across the street to the West, Sebesta Rd on the North, and Residential & Agricultural structures to the South. The Church will follow the requirements of City for screening. 3. List all incidental uses along with hours of operation (including but not limited to school, day care, sports activities, etc.) Church meetings are Sunday Morning Service(2 hours), Wednesday Night Bible Study(2hours), and Friday Prayer Meeting(2 hours).
