A public hearing isrequiredfor.a proposed conditional use permit. The applicant is
responsible for placing. an advertisementinthe.paper at least .15.days prior. to. the
public hearing. The Planning Division will prepare the add. All property owners
within 200 feet of ..subject property must be notified of the hearing. The City

2. Rec~iptfor filing fee ($25.00) plus $2.00 for each property owner on above list.
3. Ten (10) copies of site. plan which includes requirements for site plan proposals
as listed on attached sheet. This site plan will go toP.R.C.forreview,
afterwhichten( 10) copies of revisedsit.e plan will be required.
4. D~ta:iled . explanation of proposed use includinghours.ofoperation,number of
employees, number of students, children, church members,. etc., as applicable.
5. If sign is planned for. project, siteplan.should include sign elevation and
deta.ils. If. sign details, etc., .are not included onJ;Hans.submitted with this
application, any future sign must be>considered as a separate conditional use
permit and all of the above requirements ml1st be repeated.
DEADLINE: Received in Planning Division 20 days prior to Planning & Zoning
Commission meeting. .. Regular meetings are held the first & third Thursday of each
month. Noti.fy Planning Division at least3~rking dayspr~orto deadline date
