A public hearing is required for aproposedconditionalusepennit. The applicant
is responsible for placing. an advertisement ..inthe paper.. at... least 15 days prior to
the public hearing. The Planning Department "will p~eparethead. All property
owners within 200 feet must be notified of the public hearing. The City prepares
and mails these notifications to the people on the list prepared by the City's Tax
Office. See Ordinance No. 850 Section 10 for.. complete requirements.

3. Ten (10) copies of site plan which include requirements for site plan
proposals as listed on attached sheet. This site plan. will go to P.R.C.
forreview,afterwhich ten (10) revised copies of site plan will be
required.
4. Detailed explanation of proposed use including hours of operation, number of
employees, number of students, children, church members, etc., as
applicable.
5. If>signisplannedforproject, site plan should include sign elevation and
details. If sign details, etc., are not included on the plans submitted
with this application, any future sign must be considered as a separate
conditional use permit and all of the above requirements must be repeated.
