SUBJECT: 2481 HORSE SHOE DR (AA)
I reviewed the above -mentioned ADMINISTRATIVE ADJUSTMENT application turned in for
today's deadline and determined it to be incomplete. The following is the preliminary item
needed to complete the submittal so that staff may conduct a thorough review. Please submit
the following information by 10 a.m. on any Monday for review that week:
Proof of authority for Jason Sapp to sign the application on behalf of Oakwood
Custom Homes.
Please be aware that if this application is not completed before May 16, 2011, it will expire and
a new application and fees will be necessary to continue the administrative adjustment process.

I reviewed the document submitted for the incomplete ADMINISTRATIVE ADJUSTMENT
application and determined it to be incomplete. The following is the preliminary item needed to
complete the submittal so that staff may conduct a thorough review. Please submit the following
information by 10 a.m. on any Monday for review that week:
Proof of authority for Jason Sapp to sign the application on behalf of Oakwood
Custom Homes. The person that signs the application for the property owner —
Oakwood Custom Homes --has to have permission from Oakwood Custom Homes.
Examples of such permission could be the Articles of Incorporation, a resolution of the
governing board, or minutes from a board meeting where a person was expressed
granted the authority.
Please be aware that if this application is not completed before May 16, 2011, it will expire and
a new application and fees will be necessary to continue the administrative adjustment process.
